This agreement shall entitle the guest to the full use of the club facilities under the following conditions: Harbor Fitness urges all members and guests to obtain a physical examination from a physician prior to the use of any equipment or participation in any activity available at our club. In recognition of the possible dangers connected with the use of equipment and/or physical activity, member(s) and guests hereby knowingly and voluntarily waive any right or cause of action of any kind whatsoever and hold harmless Harbor Fitness, its officers, agents, employees, or instructors from any liability or cause of action arising as a result of any and all uses of equipment and/or activities that may or could occur. I understand my assumption of risk and I have been given the opportunity to review and ask questions related to my use of the facilities, exercise equipment, personal training, steam, sauna, and other equipment, as well as my participation in exercise programs or other services and/or programs offered to members. I agree to comply with Harbor Fitness’ membership policies and club rules that may be communicated to me from time to time, whether in writing or electronically, through club signage or verbally. Harbor Fitness may, at its sole discretion, modify any policy or club rule at any time and from time to time without advance notice. Guests’ pictures and personal information are guaranteed to remain private.
